How to Verify Insurance Coverage

To ensure that your insurance covers nutrition counseling services, you may wish to verify your benefits with your insurance provider. Here’s a script to guide your conversation:

Hello, I’m calling to verify my coverage for outpatient nutrition counseling services. I’d like to ask a few questions to understand my benefits:

  1. Is nutrition counseling covered under my plan?

  2. Does my plan cover CPT codes:

    • 97802 (Initial Nutrition Counseling, face-to-face, 15-minute increments)

    • 97803 (Follow-up Nutrition Counseling, face-to-face, 15-minute increments)

  3. Are there any restrictions or specific conditions (e.g., diabetes, obesity) for which nutrition counseling is covered?

  4. Is there a limit to the number of nutrition counseling sessions allowed per year?

  5. What is my co-pay or co-insurance for nutrition counseling sessions?

  6. Are there any deductibles I need to meet before coverage kicks in?

  7. Is Holistika Health Group (NPI 1407605371) or my nutritionist (Maria Ramirez NPI 1861259715) considered in-network for my plan?

  8. If they are out-of-network, what percentage will be reimbursed for out-of-network services?

  9. Do I need a referral from my primary care physician for nutrition counseling?

Thank you for your assistance.
