What to expect after I schedule my appointment?

New Patient Forms:

You will receive new patient forms, also known as an intake questionnaire, via secure email from Kalix EMR.

Please complete the provided forms before your scheduled appointment time to gather essential information about your health and nutrition goals.

The intake questionnaire is mandatory; otherwise, your appointment will be canceled.

Appointment Day:

Attend your appointment for a thorough consultation, personalized advice, and expert guidance on nutrition and lifestyle changes. Feel free to ask questions and take notes. Most importantly, be yourself.

If You Are Using Insurance for Payment:

If you are using insurance for payment, we will bill your insurance company directly.

Reimbursement Process:

If you are requesting a superbill for insurance reimbursement, please ensure that you submit your bill to your insurance company for out-of-network reimbursement according to their coverage details.

You can also use Reimbursify to handle reimbursement on your own. Visit Reimbursify directly to check your benefits and to submit your superbill for reimbursement.

Cancellation Policy:

If you cancel your appointment with less than 24 hours' notice, a $75 cancellation and no-show fee will apply.
